This Training Record form documents planned training, delivery, and effectiveness so competence decisions are consistent and traceable.

Record Training title, Training description, and Training type (e.g., awareness or refresher training).

Identify Trainer, Training date, Duration, and Trainer qualifications (Appendix 1).

Reference Attendees via Appendix 2 and include the Attendance Sheet when issuing a PDF.

Define Training objective and capture Training material plus trainer notes.

Rate Training level of risk and add a written Justification.

Select a Training effectiveness review method and define Acceptance Criteria.

Document Results (score/result) and mark Achieved / Not Achieved.

Attach Evidence of training effectiveness assessment (Appendix 3) and complete Approval, Revision, and Change History.
