This form supports consistent planning, tracking, and review of measurable quality performance across your organisation.
• Define each Quality objective with a clear scope and rationale
• Assign a measurable KPI with a defined calculation method
• Set a quantified Target and (if used) a baseline value
• Identify the Owner responsible for delivery and reporting
• Specify Data source and evidence to support results
• Set Monitoring frequency (e.g., monthly/quarterly) and review cadence
• Record Actual results, status, and Action plan when targets are missed
