Use this Document Training Form to evidence that impacted personnel have read, understood, and acknowledged controlled QMS documents (including updates) before or at the effective date.

This document is used whenever a new QSP/SOP/Form is issued or revised, and we need to prove that the right people were trained and formally acknowledged the change before the document is relied on in operations.
It ensures training is attributable and controlled. Effective date is the date from which the document must be applied; it matters because training must be aligned with implementation. Approval for release is the formal authorisation to issue the training record; it matters because it locks the record as controlled evidence.
