Your needs:

  • Training record form for ISO 13845 and MDR compliance

  • Reduce rework, improve audit readiness, and prove competence outcomes.

Use this form for any training session where you must show what was taught, who attended, and whether learning was effective.

  • Define the training scope, objective, and materials.

  • Ensure trainer competence is documented (Appendix 1).

  • Ensure attendee traceability and attendance attachment (Appendix 2).

  • Require risk level selection with justification.

  • Require effectiveness method selection (assessment/tutor/implementation).

  • Require defined acceptance criteria and recorded results.

  • Require evidence files to support effectiveness (Appendix 3).

  • Require approvals, signatures, dates, and revision history.

At Lexqara, we adapt training records to your QMS, align training program logic, and support internal audits and inspection readiness.
